UroLift® System Physician Gateway Registration Application

Register to apply or maintain participation for either the UroLift® System Physician Locator or the Center of Excellence (COE). 

These programs are intended to help connect patients with physicians who currently have demonstrated the qualifications and an interest in providing quality care with the UroLift® System.

You must first complete the UroLift Proficiency Pathway (UPP) or equivalent training in order to apply. If you have not completed the UPP, please contact uroliftcoe@teleflex.com in order to satisfy this requirement prior to gaining access to this gateway.
