Welcome to the UroLift® System Physician Gateway. This gateway houses the requirements and submission information for the UroLift® System Physician Locator and Center of Excellence (COE) programs. These programs are intended to help connect patients with physicians who currently have demonstrated the qualifications and an interest in providing quality care with the UroLift System.

The 12-month review period is based on the date the physician is added to the Physician Locator. A physician will be removed from the Physician Locator if they have not met the above requirements.
To be added back on the Physician Locator, a physician must complete at least 5 cases in the last 6 months.
Please note: Physicians who have achieved Center of Excellence designation do not need to requalify separately for the Physician Locator each year and are listed on the Physician Locator as part of their COE qualification. Should a physician not renew their designation for the Center of Excellence program, they must meet the Physician Locator annual requirements to remain listed on UroLift.com.
The UroLift® System Center of Excellence (COE) Program is designed to enhance patient outcomes and experience. Urologists included in the UroLift® System COE Program have achieved a higher level of training with the UroLift® System and demonstrated a commitment to care for men suffering from symptoms associated with Benign Prostatic Hyperplasia (BPH).
To qualify for our UroLift COE designation, a physician must:
If a physician does not renew their designation as a UroLift System COE physician, the physician will be notified, the physician’s status as a COE will be suspended, and their COE insignia will be removed from the Physician Locator. That physician will be reviewed by Teleflex to determine whether they meet the requirements to remain on the Physician Locator. If the physician does not meet the Physician Locator requirements, the physician will be notified and removed from the Physician Locator entirely 6.

1 UPP is a surgeon-training program that includes modular, didactic and hands-on device training and guided completion of 15 cases. For more information about UPP, contact your Teleflex representative.
2 Physicians who were trained prior to March 2017 received equivalent surgeon training to the requirements outlined above, including instructional didactic sessions, clinical video review and hands-on UroLift System use. The current and prior training programs focus on identifying appropriate patient candidates; preparation and operation of the UroLift System; and surgical technique.
3 In situations where elective procedures were impacted due to a national health crisis, the 15-month period for designation or re-designation may be addressed to provide accommodation. Therefore, the assessment period may not be consecutive months.
4 Excluding cases completed during UroLift Proficiency Pathway training.
5 Completion may include proctored cases in connection with a Teleflex Clinical and Medical Affairs-led peer-to-peer educational event.
6 Physicians that were removed from the Physician Locator prior to September 2018 need to meet the initial add requirements to be added back.
